Fellow hot hander here. *waves warmly*. A few things I've done to help with this is to have two bags of icing and alternate them. I use one while the other rests in the fridge or on a cold plate from the freezer. When Bag A gets too warm, I swap it for Bag B, and so forth. This does mean having duplicate tips for my most commonly used ones but I can easily be talked into more tips, anytime, anyway! :)
When piping with just one tip and one bag, I try to plan breaks in the action every 5-10 minutes as the bag warms up, then toss it in the fridge for just a few minutes. It's a good break for my hands and shoulders, too.
Lastly, I often keep a soft ice pack wrapped in a towel on my work station when piping. I'll pick it up and use it like a hand cooler instead of a hand warmer, especially when it's just me that's warm and not the whole room.
