Hi there, Annette! Even if you're using vanilla extract, you'll still need to heat the milk, then temper in the egg mixture to ensure the eggs don't cook and become lumpy, and then once thickened you can add the butter and vanilla extract. Adding the extract at the end helps to preserve the flavor. We hope this helps to clarify and happy baking!
