Hi Liz! Italian buttercream can be left at room temperature for 2-3 days before you would want to refrigerate it for longer storage. The meringue does a good job stabilizing the frosting, so unless you are transporting the cake on hot day with no air conditioning, then you should not need to put the cake in the fridge upon arrival.
