I use a cast iron camping-style dutch oven and my trick is to preheat the lid only. I find this gives a more uniform top to bottom crust. When I started using a cold dutch oven (set in the middle rack), the bottom crust cooked a bit too fast compared to the top, because the lid takes longer to come up to temperature.
