Laura, it sounds like your caramel was heated for far too long; the hotter it gets, the harder the caramel sets. Next time, try heating just until you start to see bubbles around the edge. Quickly stir it down and then work quickly to drizzle it over the caramel. Or if you want sure-fire results, you can always consider using a caramel sauce recipe which is guarantee to stay soft.
