Here is what comes to mind. It was simply under-baked. It puffed beautifully in the oven and then once removed, deflated as it cooled. Be sure the choux layer feels crisp to the touch before determining it's done. Also, be sure your oven is properly preheated. An oven thermometer can take the mystery right out!
