The timings you list should be sufficient to bake artisan breads. Most articles I have read regarding the lid suggest removing after 5-15 minutes then bake till the bread is brown or done (205-210 degrees). Finally, you might try checking your oven to see if it is correctly calibrated. You can likely find a video on Youtube showing how to calibrate your oven and ovens in general.
