Perhaps you simply heated the caramel too much, and it gradually spread. Did you set them someplace cool once you'd made them? If they were stored someplace very warm, that might soften the caramel, causing it to spread. Try heating the caramel JUST barely enough to soften so you can press it down atop the nuts.
