Hmmm, could be a lot of different things. If your yeast isn't fresh; or if you're using "rapid rise" yeast, the yeast could simply be pooping out. If you're using a different size pan than the recipe suggests, your loaf may not rise very tall, since it's spreading sideways. If your dough is overly salty or sweet, the yeast will slow down.
